A sewing table with a lift. The flat surface makes a big difference.
A really nice storage cabinet for all your stash.
All of El Burns rulers and books.
High speed internet to watch all the quilting videos.
So many notions and machines and tables and long arm.
You need to be a millionaire to afford everything for quilting.

Treat yourself to at least one book that requires a specialty ruler and of course you need the ruler

Do you have a nice reference book?
Maybe a pair of two of Gingher scissors, they have a lifetime warrenty.

How about a DVD on either your favorite techniques, or on a technique you want to learn.
Thread for use on the frame LOL

Well don't forget you will want to do a practice sandwich to start to get teh hang of yoru machine and frame soo I suggest maybe stencils you might want to try or fabric.com has some backing material that has feathers printed on it that will wash off after the quilting

Rulers rulers rulers. Nancys Notions has the olfa frosted rulers on sale. They don't slip at all and are great for dark and light fab. Bought the 6*24 a couple weeks ago and wish I had discovered these when I started quilting 9 years ago. nancysnotions.com search for OLFA I'd get the assortment!
